JIU-JITSU USA LIABILITY WAIVER & ASSUMPTIONOF RISK
Participants must sign before training.
MEMBERSHIP AGREEMENTJiu-Jitsu USA Membership ContractPayment Terms
Membership fees are billed automatically through the payment processor. Membership continues until cancelled according to the cancellation policy. Missed payments may result in suspension of training privileges.
Cancellation PolicyMembers must provide 30 days written notice to cancel membership.No refunds are issued for unused training time.
Chargeback PolicyMembers agree not to initiate a payment dispute without contacting theacademy first.If a chargeback occurs, the member agrees to pay:• original membership fee
• bank fees
• administrative costs
GYM RULES & CODE OF CONDUCTAll students must follow academy rules.Safety Rules• Follow instructor instructions at all times
• Tap early and respect taps immediately
• No dangerous or illegal techniques
• Report injuries immediately
Hygiene Rules
To prevent skin infections and mat diseases:• Clean gi / uniform required
• Trimmed nails required
• No training with open wounds
• Shoes required off the mat
Respect PolicyStudents must show respect to:• instructors
• training partners
• facility propertyHarassment or aggression outside normal training will result in removal.
MINOR PARTICIPANT POLICY For students under 18:Parent / Guardian must:• sign liability waiver

MEDIA RELEASE
Participant grants permission for Jiu-Jitsu USA to use photographsor video recordings for:• marketing
• website
• social media
• promotional materialNo compensation is provided for such use.
COMPETITION PARTICIPATIONRELEASE
Competitions involve higher risk.Participants acknowledge risks associated with:• tournaments
• travel
• unfamiliar opponents
• refereeing decisionsParticipants release the academy from liability related to competitionparticipation.
PRIVACY & DATA POLICYJiu-Jitsu USA collects participant information for:• membership records
• safety records
• billing
• emergency contactInformation is not sold or shared outside necessary services such aspayment processors.
EMERGENCY MEDICAL AUTHORIZATION
Participant authorizes instructors or staff to obtain emergency medicaltreatment if necessary.Participant agrees to be financially responsible for medical care.
GYM SURVEILLANCE POLICYPurpose
To ensure the safety, security, and integrity of the facility, Jiu-JitsuUSA uses video surveillance systems within certain areas of the academy.
Areas Under Surveillance
Video cameras may record activity in areas including but not limited to: Training areas and mat space Entryways and exits Lobby and reception areas Hallways and common areas. For privacy reasons, surveillance cameras will not be installed in bathrooms, locker rooms, or private changing areas.Recording and Storage
Video recordings may be used for the following purposes: Safety and security monitoring Incident investigations Injury documentation Theft or vandalism investigations Staff training and operational reviewRecordings may be stored securely for a reasonable period and may beshared with: Law enforcement authorities when legally required Insurance providers during claim investigations Legal counsel if necessary
Consent
By entering the premises of Jiu-Jitsu USA, all members, visitors,and guests acknowledge and consent to video recording for security andoperational purposes.
SELF-DEFENSE USE DISCLAIMERTraining at Jiu-Jitsu USA includes techniques that may be used inself-defense situations.However, the academy does not guarantee effectiveness in real-worldencounters.
Responsible Use of TrainingStudents agree that techniques learned during training will be usedresponsibly and only when legally justified.Training should never be used for: initiating violence intimidation illegal activity harassmentL
Legal ResponsibilityStudents acknowledge that: they are solely responsible for their actions outside the academy the academy is not responsible for misuse of techniques learned in training.
Students should understand that laws regarding self-defense vary byjurisdiction.
Ethical Conduct
Students are encouraged to prioritize: conflict avoidance de-escalation personal safetyTraining is intended for personal development, discipline, andresponsible self-defense.SOCIAL MEDIA
CONDUCT POLICY
Jiu-Jitsu USA encourages members to share their training experiencesonline. However, responsible conduct is required.
Respectful Communication
Members agree not to post content that includes: harassment or bullying of other members defamatory statements about the academy hate speech or discriminatory content threats or intimidation
Protection of Students and Minors
Members must not post images or videos of: minors without parental consent private training sessions without instructor approval members who request not to be recorded
Academy Reputation
Members agree not to falsely represent themselves as instructors orofficial representatives of the academy without permission.
Disciplinary Action
Violations of this policy may result in: warnings suspension of membership termination of membership privileges
